CA Blackwell and Combined Stabilisation are fined by magistrates following the death of Bernard Place in 2003.

CA Blackwell, a Colchester-based groundworks company, and Blackburn's Combined Stabilisation have been fined a total of £40,000 after a man was crushed to death in August 2003.

CA Blackwell was ordered to pay £15,000 and Combined Stabilisation £25,000 by Ilkeston magistrates following the death of Bernard Place, from Blackburn.

Mr Place died while building an additional car park for Toyota's site at Burnaston when heavy machinery reversed over him.

Blackwell Contracts admitted failing to ensure the safety of persons not in their employ.

Combined Stabilisation admitted failing to provide a safe system of work and failure to provide safe plant.

Putney based construction company Henderson General Services Ltd has been fined £18,000 with costs by the Old Bailey following the death of 63-year-old painter James Grimes.

Mr Grimes, of Southfields, fell from a ladder while attempting to access a scaffold in Queen's Gate Terrace, London.

Henderson General Services Ltd pled guilty to breaching section 3(1) of the Health and Safety at Work Act 1974 after a prosecution by the Health and Safety Executive.
